Beyond the Blue Zones: What 43 Years of Italian Data Reveal About Living Longer


The term "Blue Zone" has become almost a household phrase, shorthand for a handful of famous places around the world where people seem to live remarkably long, healthy lives: Sardinia, Okinawa, Nicoya, Ikaria, Loma Linda. It is a compelling idea, but it can also flatten something more complex into a short list of five dots on a map. A 2025 study published in Nutrients, by Silvana Aliberti, Daria Nurzynska, and Mario Capunzo, takes a much wider lens, examining longevity trends across all twenty Italian regions over more than four decades, and finds a more layered, more useful story underneath.

A 43-year national dataset, not a handful of anecdotes

Rather than studying one famous longevity hotspot in isolation, the researchers pulled national demographic data spanning 1982 to 2025 across every region of Italy. They tracked several specific longevity indicators:

  • Aging Tendency, a measure of how a population's age structure is shifting over time

  • Longevity Index (LI%), the proportion of the population reaching advanced age

  • Centenarity Index (CI%), the proportion of the population reaching one hundred years old

  • 85+ and 90+ ratios, tracking the proportion of the population surviving into very advanced age brackets

They paired this with age-standardized mortality data for five major non-communicable diseases (NCDs), the chronic conditions responsible for most deaths in aging populations, cardiovascular disease, cancer, diabetes, cerebrovascular disease, and respiratory disease, tracked from 1990 to 2023. Finally, they layered in contemporary behavioral and nutritional data for adults aged 65 and older, collected in 2024.

This kind of long-range, whole-country dataset allows for something the popular Blue Zones narrative often cannot: looking at how longevity actually shifts over time and correlates with measurable, trackable factors, rather than relying on a snapshot of a few exceptional communities.

The clearest finding: national longevity keeps climbing, but unevenly

Every longevity indicator the researchers tracked rose steadily across the more than four decades studied. Italy, as a whole, is aging and living longer. But the gains were not distributed evenly. Northern and central Italian regions consistently showed the highest longevity indicators throughout the study period, reflecting real, persistent regional disparities rather than a uniform national trend.

What actually correlated with living longer

When the researchers examined which factors were most strongly associated with higher longevity indicators at the regional level, two specific causes of death stood out. Regions with lower long-term mortality from diabetes mellitus and cerebrovascular disease showed the strongest correlation with higher longevity and centenarian rates. In other words, the regions doing the best job managing metabolic health and stroke risk over the long term were the regions where people were most likely to reach advanced age.

Nutritional profiles also tracked with this pattern. Regions in the north generally showed more favorable nutritional indicators among older adults, aligning with their stronger longevity outcomes.

The most interesting twist: Cilento

Here is where the study becomes genuinely compelling rather than simply confirming what a north-south divide might predict. Despite the overall northern advantage, one southern area, Cilento, a coastal and mountainous region within Campania, emerged as a clear outlier. Its longevity indicators were comparable to Sardinia, itself one of the world's original, well-established Blue Zones, and above the average for its broader macro-region.

This matters because it breaks the assumption that extreme longevity in Italy is simply a north-versus-south story tied to broad regional wealth or infrastructure differences. Cilento demonstrates that favorable longevity outcomes can emerge from a specific, localized combination of factors, even within a macro-region that does not otherwise stand out. The researchers frame Cilento as a genuine southern hotspot worth further, dedicated study, and this study builds on the same team's earlier work exploring whether Cilento shares meaningful similarities with officially recognized Blue Zones.

Reading this study honestly

It is worth being precise about the type of evidence this is. This is an ecological study, meaning it examines correlations at the population and regional level, not at the level of tracking individual people's lifestyles or diets directly. The associations identified, such as the link between lower diabetes and cerebrovascular mortality and higher longevity, are described by the authors themselves as hypothesis-generating rather than definitively causal. This is a strength in the sense that it reveals broad, real-world patterns across an enormous dataset, and a limitation in the sense that it cannot tell us precisely which individual behaviors are driving those patterns for any one person or family.

Why this reframes the Blue Zones conversation

The popular version of the Blue Zones story tends to treat longevity hotspots as fixed, almost mythical places, as though the secret lies in being born in exactly the right five regions on Earth. This study suggests something more dynamic and, in some ways, more hopeful: longevity outcomes shift over decades in response to changing patterns in chronic disease management and nutrition, and new hotspots like Cilento can emerge even in regions not historically associated with extreme longevity.

The practical throughline across the data is consistent with what other longevity research keeps surfacing: managing metabolic health and cerebrovascular risk over the long term, paired with a genuinely favorable nutritional pattern, appears repeatedly as a marker of populations that live longer. That is not a mysterious secret confined to a handful of famous places. It is a pattern that, at least at the population level, can apparently take root wherever the right conditions align.
